To have Jerry live just never use him. To get an A+ get, but do not use, all three items and play the whole game through without dying once.

new game, leave default name, select gunslinger as a class, skip turns on the whole 1st battle until you die
reload the save and press the red circle when you get to the last map/area then skip turn, enjoy your badges ^^
