It seems that for Lonely Wolf achievement, you cannot give Bill the painkillers. On my first try, I gave them to him, and didn't unlock the achievement, however on my second try I redid everything exactly the same way, except for givin Bill the painkillers, and got it.
Well, I finished the "hard edition" first, then I turned this game on and... noticed that all the levels are unlocked and marked as completed. So I guess these two games share a savegame file...

To all IE users: if game gets laggy, try using another browser (FF for example) - flash player for IE seems to be much worse optimized than the one for FF and almost all games on kongregate are lagging on my computer when playing them in IE, but not too much when playing in FF.
