the both regeneration pills and the surgeon drone malfunction when spawned in on pre-built levels, spawning in random body pieces rather than the ones actually missing
normal mode is pretty easy, hard mode is just the game stabbing you in the leg and telling you to deal with it, and insane mode is just the game handing you a grenade and pulling the pin.
