You need to specifically manually save to store loaded Custom Items locally (otherwise the list would fill with *all* the stuff you load, and bloat your save file needlessly.) You can do this via 1) save icon in the Spawn Menu while a loaded custom item is selected 2) "Manage" button in [Library > Custom].
