Spoiler Alert!
For those interested in getting the Swaintastic Badge and are too lazy to figure out which levels require loss of life, here was my strategy. Lose 1 life in the following levels: 3, 4, 6, 11, and 18. Lose 0 lives elsewhere.
I imagine that there are other ways to do this, but this is how I accomplished it.
