died after finishing first quest, then returned to save file, with the quest still finished, but having to re-accept it. Everything else was the same.

came back after 18 hours to find that my save file is gone... was through 37 levels...

Notice how the arrow adds a moving hand when it is over the picture of the woman on the first floor...
