Ooops. Just realized it took forever because it had defaulted to easy rather than normal! Replayed on normal -- reasonable amount of time required, and got the card just fine.
Points gained so slowly, damage taken so slowly...just annoying that it takes so long to get the card. I type very well, but it just took too long. Finally had time to sit here typing forever and got over 25000, then waited forever again for them to destroy the temple...and I didn't get the card!?!?
Glitch: Second area, start w/ lead pipe but clicking mouse doesn't swing it. Hit space to change weapon, but that just throws out this ever-changing weapon in front of me, doesn't change the pipe. Died, now the ever-changing weapon is in my hand in the menu page too. Tried reloading, same thing. WTF?

One question: what is the time required to beat each level under to get the lightning bolt? Is that posted somewhere & I'm just missing it? Would be nice to know what time I'm wanting to beat for each level...
I agree with redfreak about the time/clicks required to quit hole. It can be pretty annoying when you're doing the same round over & over & over trying to get the medal...
Also convenient (but not necessary) would be the ability to "reset" hole, or perhaps a way to play the holes as practice, without scoring.
