I can't remember which one (dying from the big jet and falling from the very top jets into the pit) is Stalagmite or Stalacite.

hideouts simple first shoot the factory pipe releasing green gas shoot the person on the top left corner then shoot the guy below him outside the building while the one inside isnt looking then shoot that guy then shoot the other guy inside the building then shoot the supporters on the electric pole then quickly shoot the last guy
and for the ppl who dont know how to get "Jokes on me" medal, you get it by finishing the "you must finish this level by __ seconds or you will be taken back to the title screen" before the__ gets to zero.

at the start, u have to go to the complete other side of the trophy blocking hole. the wall of death comes from the left, so go to the right side of the hole.
The title trasher is earned by clicking the face at the very top right. It's hidden. Do everything it says, and it'll soon take pity on you and give in.
