at the start, stay on the right and avoid the spikes, they'll stop before they crush you, then proceed with the real game.

For Still Alive Badge in the room before the snake room, where it says "You have done an extremely well in in reaching the final level of this game..." Go the the left and you'll fall through the floor and go trough a secret area. Grab the trophy, and go through a final room, and you'll get a nice cinamatic, too.

For title thrasher click the right top corner of the title menu, on a frowny face. Do everything it says to do, and you'll get the badge.
