Seeing "code ads" (HTML instead of an image)? Let us know details here please!

A few users have reported seeing code-only ads like this (courtesy of microchip08):

If you see one of these, or have seen one recently, please post here and give us some information so we can try to track it down:

- Operating System (including version if you know it)
- Browser(s) (including version, especially for IE)
- Any unusual or aggressive plugins (like AdBlockPlus, NoScript, FlashBlock, etc.)
- How often does it happen?
- If you can, grabbing the embed code (see instructions here) with Firebug or another inspector tool would be very helpful so we can track and see if it’s a common ad provider or not.

I’m not sure if this will help, but about 5 minutes after the maintenance I saw that instead of an ad.

It has been appearing constantly for a few days.
It’s a few days old vista instalation. It does work on my mac laptop/safari.
