A couple of things:

How is this different from other games like this? The ambulance is a new idea, I think, that once you're over that, it is just like a lot of others, but with less variety. There are now powerups or upgrades available, no turbo or nitro boost, etc. Lots of similar games have those.

Second, I am always annoyed by keyboard-only games which then make you reach for the mouse to start a level, reset, and/or go to the next level. Especially with yours, you do not give a 3-2-1 countdown (or something) to start, so if I want to use my right hand on the controls, I have to reach across and click the mouse with my left hand or else I'm already behind in the game when it starts. I would recommend both offering keyboard ability to start a level, AND have some sort of lead-in once you are in the level but before you are able to start moving. (A message like "Pick up patient at 4th and Vine!" for 2-3 seconds would work.)

Firstly I was immediately impressed with the graphics. They are not astonishing but they really create and capture the changeable environments well. It helped with the frustration on some of the later levels. However I discovered problems with the physics early on. When I reversed on a straight section and accelerated I tipped over my back completely. Apart from that though the vehicle does feel controllable and it is somewhat fun to drive. One of the major issues your having though, and you'll need to fix this, is a scripting error with certain sections of each level in regards to backgrounds. At the start of some levels, and somewhere halfway, the entire background layer is lost. However I think you've put some effort into this. The mechanics need some fine tuning, but the game is playable, enjoyable and a little memorable. What would really boost this experience entirely is some energizing audio.

have a bar to tell how bad the guy may be paralyzed and controls need to me d and a are forward and reverse and w and s are for pitch and gravity needs to be a bit stronger the paitient tends to go to far and the back of the truck needs to have a slightly taller tailgate so the paiteint doesnt fall out as easily
