This is a good game idea. I enjoy it to some extent but it is somewhat strange when the camera wigs out and the fact that you can't tell building to flat outlined land

There should be a minimap, cause right now it's just trying not to get lost in the streets. Also, make the buildings look more like buildings. I couldn't tell the difference between them and parking lots (or whatever the big grey spaces are). And finally, tweak the peds. They'll run in the street for no reason, and I highly doubt running into a stationary ambulance will ever kill you (which is exactly what happens when I'm stopped to pick up a patient).

This is really hard! A tutorial would be real useful, I gave up after not really understanding what I had to do.
