*SPOILER*
The girl does not represent drugs. She is "Self-Mutilation." Notice in Ending 1 when the nurse says "You're lucky you didn't hit any nerves or arteries." In this ending you also become "Closer" with her (deeper cuts). She also has "wrapped herself around my arm and never left" (scars).
If you play through the game again with this in mind it all starts to make sense. The game sends a great message about the dangers of self-mutilation, and is also very well put together. The music is also very deep and relaxing. 5/5 for sure.
