You can only get a specific Item once. There are Rare/Legendary versions of every item that are more powerful. In your backpack, you will notice 3 treasure chest icons to the left of the zone page name. You can toggle each of these to show you which magic/rare/legendary pieces of gear you have collected.

There is a small chance you can find rare items in the “normal mode” zones. Hard mode is where you are really going to find all of that stuff. It is accessed through the portal near Bridgetown. Putting on +Item Find gear, slotting Item Find charms and, of course, purchasing Potions of Finding in the game store will all boost your chances.

Actually, Procs activate when you use the ability on the piece of gear they are on. For instance, if you have 30% Lifethief on a Saber, every time you use Quickstrike, you have the chance for the proc to go off. It’s the same with charms that have +Ability and +Proc features. They only affect the ability of the item you put them in.

This also mean you don’t want to put those types of charms into gear that don’t have abilities.

Actually, in hard mode that is a common ability on Lieutenants and higher type mobs. You can get to hard mode through a portal in Bridgetown, just after the Swap Road zone. Be warned, its not called hard mode for nothing ;}
