Sell off any starting symptom that you may have, you need to have zero visibility, bonus for you if you manage to get some level 1 resistance.
Acquire all level 1 resistances, you don’t need to rest, after that, just accelerate time until you have successfully infected all regions.
Just wait, and if you are successful, you will infect all regions, Madagascar included, this may take a few tries.

Man, it's so much easier in relaxed mode. It doesn't matter where you start, just start as parasite and sell all the symptoms untill you've got everyone infected. Then buy all kinds of crud like hemoraging and people die like nuts. I don't think this'd work for the assasin badge, because it takes forever. But the population is really low right now, only 253 living people.

Wow, at first I thought this game was kind of cool and gave it 4 stars. Then after trying the strategies listed here and various other places and not even beating relaxed much less realistic, I realized that this game sucks. It takes too long, is too hard (if you count a 1 in 1000 chance of infecting everyone, hence its only a luck game), and just gets repetitive and boring quite quickly. 2/5 only because you kill humanity (or try to).

Frustrating, yet strangely fun game. Once you have a decent strategy, it becomes a game of luck. It all comes down to trying to infect Madagascar before those paranoid schizos shut everything down to prevent themselves from a sneezing epidemic. My last try I missed the final badge by 2 days, very irritating. Oh well, I guess I just need to try again. Although it's initial appearance may seem to be complicated, it's actually a very simple game. The game has many variables to try and keep each game unique, but there's not enough player interaction in my opinion, to achieve that goal. It has a much greater potential than the developers are taking advantage of. 3.5/5.

Great fun, although a bit odd - by day 80 on Realistic there were 50,000 people left on the planet and my disease had an average death rate of over 50m/day. Those 50,000 then took another 48 days to die. Tenacious little sods.
