I just played a game and kept a close eye on madagascar...
In the 22 days it took for Madagascar to close it's port, only ONE ship came in from anywhere in the world. Yeah, no wonder it's so difficult to infect.

The guides you do see it do work, but don't limit yourself to the following islands, Madagascar is still a clean island to start off, but you need to regulate your infectivity from time to time. If you find that Madagascar turns to a hue of red before it can infect other countries, take off your sneezing symptom to slow down the clock to closing borders. I hope this addition to advice helps, bump it if it works.

My disease never spread so I just totally annihilated Peru. Also major lulz when ships are out at sea and then all the ports close.

Impossible badge for Pandemic 2: Pick virus, start in new zealand, cuba, greenland. Sell all starting symptons apart from sneezing, and if you dont have sneezing, buy it. Get your resistances to 1-1-1-0. After you’ve infected a few countries sell sneezing, as in 3 countries. Wait till all countries are infected, if this is after day 30, restart. Get lvl 4 drug resistance and buy coughing, sneezing and vomiting. Wait for 5 days on the game and buy fever, fatigue, diarrhea, pulmonary edema, hypersensitivity, hemorrhaging Wait.

I started on Madagascar, and it took a while, but I managed to infect everyone but Japan. Noooooo!
