The rarity of infection of Madasgascar is a flaw. Takes all the fun out of the game if you infect every country and have to wait 20 more days while it is never visited, then it closes its borders and you have to start over.

Once you have infected all the regions, acquire all drug resistance, up to level 4. Then acquire Sores, Vomiting, Boils and Necrosis.
Once you acquired some lethal symptoms and started to kill off the population, the remaining number of active hospitals will be quickly shut down. Hence, the vaccine will never be completed, never be deployed, thus your path to global wipe out is eminent.
Accelerate until the number of healthy population is zero so success is in hand. Only need to make sure that your web browser or computer do not suddenly crash.
Soon more Countries will appear in forsaken regions.
Now just buy heart failure or the long one begginning with E (Enchupitis or something) and wait till everybody dies.

Choose parasite as your disease class, as you need to make sure no one notices your existence while you slowly infect all regions the soonest possible.
Sell off any starting symptom that you may have, you need to have zero visibility, bonus for you if you manage to get some level 1 resistance.
Acquire all level 1 resistances, you don’t need to rest, after that, just accelerate time until you have successfully infected all regions.
Just wait, and if you are successful, you will infect all regions, Madagascar included, this may take a few tries.
