this is my guide:
1- pick disease type (virus, infection, bacteria)
2- unselect (sell) any symptom and any transmision way.
your disease will be completly stealth.
3- upgrade cold-heat-moisture lo lvl 2 and then drugs 'till lvl 4
now patience my young padawan...
4- wait 'till every (i mean every) country gets infected. if anyone closes both airport and shipyards without being infected, you will need to restart.
5- upgrade every transmision way and make your disease as fatal as posible.
6- good luck! you might need to try several times (maybe 15 times...)

I killed 100% population using parasite. Little luck involved.. as madagascar closed it's port while one last ship was already in route and to my suprise, apparently ships already en route and almost there are allowed in.
The problem.. Kongregate failed to successfully record my achievement. I am not typing this to complain or whine... but this is a warning. If you win the president of Madagascar medal as I did... TAKE A SCREEN SHOT (which I did not!) No screen shot.. no medal.
Fix the glitch that deprived me of glory!!!!!

Basically the theory is to keep transmission slow enough at the start to give the disease time to spread to every single country before they shutdown. The faster your disease spreads, the faster the shutdowns I think.
