This method has worked on realistic/relaxed so far even though I started in Canada! Parasite>Sell any symptoms unless it comes with fever>heat 1>cold 1>moisture 1>waterborne>drug 1>drug 2 ~lots of patience~ Pick head/cold/moisture 2 if the last few regions need infecting are all one of those. Once you have every region infected, start buying symptoms. They'll start researching a vaccine so buy drug 3 and 4 and continue on to infect/kill everyone :)

Used the guide in top comments, started in Canada. They didn't spread the disease in the first 30 days. Damn Canadians being so nice...

madagascar suffers from wide spread drought gets hit by 2 earthquakes and after shocks still refuses to open ports to allow supplies for their "country"

I got a few ideas what if we can upgrade the symtoms or/and see the victim with all the symtoms or an option to get vaccine immunity if you max out every symtom transmission and resistance. :)

1.) Pick Virus. 2) Start in Canada, New Zealand, Cuba, Greenland, West Europe, or Madagascar. If you do not get there restart. 3) Sell the starting symptoms, and buy sneezing (unless it is what you started with, just keep it). 4) Buy 1-1-1-0 resistances but no transmissions (they trigger closures). 5) Once four countries are infected, sell sneezing to get rid of visibility. 6) Wait until all countries are infected. If Madagascar closes its shipyards/borders or is not infected in 30 days, restart. 7) Buy all four drug resistances AND sneezing, coughing, and vomiting. 8) After a few days, unlock tiers 2-3, (do not buy tier 4 ever!) keeping whatever symptoms it gives you. 9) Buy fever, fatigue, diarrhea, pulmonary edema, and hypersensitivity. Then save for kidney failure and ataxia. 10) keep this comment alive Thanks for the comment! I got 13th of all time by using this method. Almost worked for me will keep trying Thanks!

Started in Madagascar, infected nearly everywhere... then my disease spread like wildfire... that is until both Central and South America decided to go on lockdown...

Love games that take some skill this is all about luck if madagascar closes the game is over. period. there is no winning.
ANd parasite after 30 days i had 14 infected with tier 2 across board.
Waste of time.

here's a glitch- all the ports can be closed but there can still be ships out there. they will end up going to a closed port. its how i got madagascar :)
