I start with Madagascar. Then no other country gets infected. Am I meant to believe that Madagascar is completely isolated?

Just do what Nauto98001 stated. It's a lot of restarting and luck but eventually you'll get the right combination. Greenland worked best for me, with 80 days to extinction.
A little hint: When you do it right, noone closes any borders, airports or shipyards before they are infected. <=Just a matter of finding the right time to get rid of sneezing.

I know why Madagascar is so tough to infect!
The person who engineered it lives there and just sends it by mail to some other country and tells the President of Madagascar what he did and thus the PM shuts down the entire country!
