Have an idea of how to get it now, based on this: if you have Fever for long enough as a symptom, you gain the "Ablaze" trait, which is basically super-fevers. There must be a symptom that becomes Head Popper, as well.

final verdict: the idea is really cool, but in the end it's a game which requires almost 0 skill and tons of luck, which is incredibly annoying.

It's fun, but there's absolutely no skill involved. It's just about picking the right starting country (and restarting if you don't start there), not buying anything until you're bored to tears, and lucking into a ship arriving in Madagascar at the right time. To call this a strategy game in the real sense of the word is ridiculous. But the interface is cute and there's some minor replay value to be had. 4/5 -1 for being about blind luck = 3/5.
