The trick is to control speed of infection for your virus. Most countries start to export virus when the infected population reach around 130000 mark. The government start to notice you when you reach about 3000000 mark at the starting region. So, the time left for you to infect the whole world including madagascar is between those two marks. All you need is patience and time.
In order to control the spread speed, you need to play around with resistance combinations. Each virus type has a special combination for my stretegy to work so test each of them to see which one works best. Magic formulae for parasite is 1-1-1-2~4 Resistance. Just wait for long and the world will not notice you until you have infected every region. Once you finished this stage, you can just upgrade anything deadly and the world will fall.

I put in the disease name as 558555 and got Madagascar everytime, realistic and relaxed. If it works for you + this!!!

I guess from what my disease did that the commonly know disease The Sniffles Kills 100% of the population (Yes, you too madagascar)

This would be so much better with a restart button instead of having to go through all of the menus to begin a new game. Or the option to choose where to start from, namely Madagascar.

around day 60 i look around and see every place expect Cuba, Madagascar, and Indonesia are infected in the red zone i look over to Greenland and they have no healthy people and are the only ones on the planet with an open air and sea port... makes me wonder whats happening up there

A small alteration of Nauto's stragegy is to not buy sneeze but buy resistances up to 1-1-1-1 and then just wait. That worked for me. (although it was a close call. 7 days more and I'd lost.)

I used Nauto9800's methos in his/her apr21 2010 post, but with brazil, and that works fine too 'cause the population density is so high there
