Madagascar isn't that hard to infect, I've beat the whole game without it being my starting region or anything. Just make your virus have a low visibility rate, and buy most of the traveling early. Seriously, the game isn't all too hard..

Easy Walkthrough Pandemic 2:

First, choose relaxed and pick a Virus, name it whatever. You may have to do this repeatedly,
but keep restarting the game until your disease arrives in either Cuba, Madagascar, or New
Zealand first. Because they are small countries, it will be harder for the world to develop
a vaccine quick enough. Then, sell all of your disease symptoms and buy all level 1 resistance.
Make time go as fast as possible. After about 5-10 minutes, you should have infected the entire
or at least most of it, and have around 140-150 evolution points. It is particularly hard to
infect Argentina, Peru, and Madagascar. After you have done this, buy Airborne and Waterborne
transmissions and all the drug resistance. Finally, buy all tiers of symptoms and (you only
need these two or three symptoms) buy: FEVER, ENCEPHALITIS, VOMITING, AND ANOTHER OF YOUR CHOICE.
You should beat the game in about 10 minutes.

Start as a virus and when it asks you to name your disease delete what is in the box
so it is completely blank. than start the game. There is a good chance you will have
started in madagascar, if you didn't start in madagascar try again.

1.) Pick Virus. 2) Start in Canada, New Zealand, Cuba, Greenland, West Europe, or Madagascar. If you do not get there restart. 3) Sell the starting symptoms, and buy sneezing (unless it is what you started with, just keep it). 4) Buy 1-1-1-0 resistances but no transmissions (they trigger closures). 5) Once four countries are infected, sell sneezing to get rid of visibility. 6) Wait until all countries are infected. If Madagascar closes its shipyards/borders or is not infected in 30 days, restart. 7) Buy all four drug resistances AND sneezing, coughing, and vomiting. 8) After a few days, unlock tiers 2-3, (do not buy tier 4 ever!) keeping whatever symptoms it gives you. 9) Buy fever, fatigue, diarrhea, pulmonary edema, and hypersensitivity. Then save for kidney failure and ataxia. 10) keep this comment alive Thanks for the comment! I got 13th of all time by using this method.

I got lucky and somehow got madagascar without starting there and got the last badge by using the guide said under. Had around 19 days to spare, mostly luck, just going to say.

i started in mexico, sold all symptoms, bought 2-2-2-2-2, rodents, airborne, and waterborn, wait till everyone was infected, then completely dehydrated everyone. Fever, kidney failure, liver failure, vomiting, diarhea, dementia.

I had no problem beating this game up until day 99 where as I had 130 or so people left and they didn't die. So i missed the "Impossible" badge by 1 day or about 100 people. Sadly I will never attempt that badge ever again for it only leads to frustration. :P
