I am trting to unlock batara kala's last spell and I'd like to devise a strategy to maximise plague spread.
I get it does trasmit by proximity and trade routes, but I'd like to know the respctive probabilities.
Also, does Health have a roel in this?

Health has a role (lower = more vulnerable), as does the number of plague doctors present in a city (each helps reduce the chance of plague spread).
The spell will simply affect the target.
Transmission as far as i know is automatic, consider the health/plague doctors to provide a "saving throw" against being affected, though I don't know any mechanics.

Interesting, so in a Batara Kala run it maybe worth to avoid wells/aqueducts/hospitals. That could make your health easily reach 0% tho, does 0% health cause your citizns to die?

You are right that plague can only spread to close cities or via trade routes.
A plague have more chance to spread to a city if that city has low health and high population. Risk is also reduced the more doctors you have in it.
If you want to increase the plague propagation, I would recommand spamming trade routes to infected cities and once your cities are infected, switch them to healthy cities.
And yes, your citizens would die from disease if you have low health except if you have a high enough level in the Ruler of the Underworld spell. This spell will allow you to have poor health and keep your citizens alive.

you can't get lower health by just not building the health buildings: your citizens will just die and you dont get significantly lower health since less citizens = more health. If you want cities to be excelent plague spreaders make sure they have max size for their health buildings (wich gives you around 30-40 health without plague) because larger unhealthy cities keep the plague longer. Also fire all plague doctors so the plagues in your cities last longer. Combine that with the trade route strategy suggested by to_change_later and/or by going to war with empires that have plague close to them and kill of their plague doctors in attacks.
