This has happened to me a lot. It is just a visual error. If you look at the bonuses on your weapons, they sometimes change after the second chest, meaning that you did pick up the weapon, the animation just cut out to early.

On Hero Screen the Dwarves Charge Skill still says +2 instead of +3.

Sometimes some creatures don't show up visually in a room until after you've taken an action. Just a moment ago, I stealthily entered a room, and two monsters appeared. I shot one of them with my bow, but a moment later, two more monsters popped up adjacent to my character and attacked him with the defense penalty from using the bow.

I don't recall specifically. It was during a daily hunt, so not the specters; I haven't unlocked those yet.
One other minor graphic bug I just came across, not a big issue: In the ruins, fighting 2 mummy spellcasters with 1 health each, they both prep fireball. I defend. The first cast fireball goes off, killing the other mummy. The explosion graphic followed my character around the room as she picked up the scrolls and loot. It didn't change the gameplay any, though, and it didn't stick around when I entered another room.

It was probably ghosts. They randomly come back after you hit them. They only stay gone if you rid the room of all ghosts.

Yeah, that will happen. It's sort of amusing, so we haven't bothered to fix it.

Since this is a daily hunt you can't just stealthily enter a room with ghosts already hit (unless you're actually trying to do so). By the same reasoning, they weren't spellcaster summons.
To me this sounds like a case of the monster sprites failing to load and thus being invisible until they turned to attack the player, but I think the game actually tries to prevent such glitches as I've never encountered this in all my experience of playing on lousy wifi.

It was not ghosts reappearing, because it was the first action I made after entering the room.

Not sure what's going on, but i'm unable to select the bow option for the dwarf. The button is there, but remains greyed out. It has been like this for the past few days.
Edit: It's working today.

The only time my dwarf can use the bow is 3 tiles away or 2 tiles when blocked. I've never seen a 2 tile clear bow option even show up. Sure would be nice to have.
