The Surgeon cannot control his fingers properly, unable to hold things, drops tools at the bad time *Cough* When you are getting the good heart out and you drop it on the floor *Cough* And can make his whole hand to a 180 directly. To be honest, I think the person being transplanted on can do his job for him, On him.
