To get Madagascar and win the easy way:
- Make a Parasite that is COMPLETELY invisible (no spreading or symptoms upgrades).
- Upgrade drug resistance to at least 2-3.
- Upgrade all other resistances to at least 3.
- Collect evolution points until the first preventive action is taken by some country (maybe later if you don't have enough for the next step)
- Do a full-on breakout (airborne, waterborne, rodent and insect, where water is the most important).
- Upgrade resistance (still no symptoms).
- After everything is upgraded, start randomly giving symptoms.
By that point, the world will be pretty much infected...
