Diseases with Chest pain as a symptom come up a lot in the beginning by design. It's to get you thinking and realizing you need history and exam courses to figure things out. If the diseases at the start all had different unique symptoms it would be incredibly easy!

I don't know what happens after level 7! I think it will kinda break. I updated the game so you can basically play it forever now. I never made it to level 7 in anything so I never considered that! Ha ha!
nil means "nothing". You'll find out more in exams as you unlock more of the exam tools. As you get more diseases you'll need more of them to figure out what is what.
Thanks =). I have a lot of stuff planned for version two but figured the game's fairly long enough as it is for now. I have some big plans for both content and fun-factor though planned for the next one! I'm really hoping for badges. Hope Greg likes it =).
memmott - just diagnose a couple diseases just from the first symptom. You go to the book to guess a disease. After you're level 2 in any category you'll become a med student =).
Oranje - you need to unlock the different exam techniques. There's lots to find for each disease. As you unlock more it gets harder to figure out what is what without using the exam techniques.
