There's only a small chance of spawning an actual physics droplet, which is able to stain things it touches for effect. Most blood particles don't have physics bodies attached to them because spawning thousands of tiny objects would lag everything up way too much.
There are currently 240 items to play with, 130 of which have unique functionality. (That's at least three times as many items as MaD1, and probably 10x the uniques.) The expansion has 20-40 new items on top of that, so please try to manage with the massive heap of existing items while I work on the expansion. Jeez.
