Keep playing the insane dungeon till you find a champion called failed diablo clone and once you kill him you get the secret dungeon and the hardest monster in the game is in that dungeon
i had some probems getting the death one too
cause i kept on dieng and sometimes i survived but i didnt get it
but i got it yesterday so just keep on trieng heaven
