Use the empty MP3 player on the store attendant (when facing her, use it from the inventory by selecting it and pressing space), and then on the girl by the racks, and then back on the attendant. Go to the restroom. Check the sink in the restroom for a drill bit. Use it on the wall on the right of the payphone outside. Examine the payphone.

As others have pointed out, the charater seems to remember having visited the doctor before because at the end of the end screen (past the tester credits) there is a white message that reads "The shrink leaves, and suddenly I don't even have a person to tell my story to anymore." My guess is that he wants to tell the story again and again because he just can't get it over with.
Always feel good to hang out at the Philosophical tag, and this game enjoys a well deserved highest rating in puzzle category. But seriously, we need a tag to store games that have beautiful and sad storylines and good music, like this one. For those stuck at the two pit levels, try blind jumping while holding right. Eventually you will get a couple of shadows to get across. In the first level (green beam), keep jumping even if already fallen or on the other side, because shadows need to be stacked. In the second level, do not press spacebar to move on to the next shadow until you are confident that the current one would have reached the exit if it had not fallen, because there is a chance that one of your previous shadows can randomly get across, and shadows that reach the exit count.

Cart sometimes make unnessesary reroutes, e.g. passing through a pick-up point loop near the main road. This can be a problem if two pick-up points are both occupied. Also, chars wait too long before rerouting and too long for routing.
Free staff occationally does not immediately go to a empty rack to refill it, especially when lots of racks are empty. Racks that share walking spaces with registers never get refilled. Customers report "out of stock" when only some of the racks of a genre is empty. Fast forwarding has no effect on customer coming rate.
