you need a maximum of 15 dmg to get the gold star. I recommend trying to get the golden stars for enemies destroyed and health at the same time, then trying the time ones.

Nevermind, i found the "other" ending, yet it seems a bug is getting on the way...
I completed it at around 98%, then 100%, the endings didn't change... Was that supposed to happen? :S

How to deal with the multiple battles bug? go to the boy when you wake up and press DOWN to talk to him BEFORE you read all the other text.
Having to beat the nightmare 3 times in a row got to my nerves... 3/5. Could have been a 5/5 but the bugs are too annoying.
